[vc_row][vc_column width=”1/4″][vc_widget_sidebar sidebar_id=”custom-sidebar24″][/vc_column][vc_column width=”1/2″][vc_text_separator title=”2016 Community Health Improvement Plan”][vc_column_text]The Columbiana County Health Partners began conducting community health assessments (CHA) for the purpose of measuring and addressing health status. The most recent Columbiana County Health Needs Assessment was conducted in part by Strategy Solutions, Inc. (SSI) with assistance from the Columbiana County steering committee members. The CHNA was composed of both primary and secondary data. Primary qualitative data, including in- depth stakeholder interviews, focus groups and surveys, was collected specifically for the CHNA to reflect input from the broad community and vulnerable populations. Secondary public health data regarding disease and mortality, as well as behavioral risk factors, was gathered from numerous sources, including the Healthy Communities Institute, Ohio Department of Health, the Centers for Disease Control and Prevention (CDC) Healthy People 2020 and County Health Rankings. This has allowed Columbiana County to compare the data collected in our CHA to national, state and local health trends.
